Office Policy

SeaSide Integrative Medical Center is devoted to helping you achieve optimal health and vitality with a personalized treatment plan. It is important to read all the information that was sent to you carefully. All intake forms and consent forms must be completed and mailed or faxed to our office at least 2 days prior to your appointment. This will allow us to make the most of your time during your visit with us which will in turn give you the best quality care.

Consultations
 
Your first visit includes a 60 minute medical consultation with your physician and a 50 minute nutrition consultation. This nutritional consultation is a vital part of your Functional Medicine Program. At this time your physician will decide what testing or additional studies are to be completed. Nutritional therapy and laboratory/diagnostic testing are essential in determining the root cause of your problems and in the development of your treatment plan. Nutritional supplements are a much needed part of your program and are typically suggested.
 
Office Visits & Pricing
 
If you are coming from out of town, you may need to stay overnight and have your blood drawn the next morning. Many of the tests require a 12-hour fast. You should drink water during this fast. The staff will review the costs of all testing after your medical consultation and before labs are obtained. The purpose of your follow-up visit is to review and interpret the results of your testing and further develop your treatment program.

Confirmation and Cancellation of Appointments
 
In order to accommodate every person’s schedule as much as possible, there is a 2-day cancellation policy, and your appointment must be confirmed at least 2 days prior to your scheduled appointment. You may cancel your appointment by calling our office.
 
Payment Options
 
Our office accepts cash, checks or credit cards (MasterCard, Visa, Discover) for services rendered. When you schedule an appointment, we request a credit card on file to hold the appointment for you. We will NOT charge your credit card unless you miss or cancel an appointment without proper notice. When you come for your scheduled appointment, we will itemize and review all of the charges for consultations, laboratory testing and nutritional supplements. Payment is due on the day of service. Follow-up phone consultations will be billed to your credit card on file in our office unless you provide other payment information and instructions prior to your appointment. If additional lab tests are required and our office sends test kits, the appropriate fees will be charged to your account.
 
Insurance Information
 
SeaSide Integrative Medical Center does not accept insurance or Medicare. We cannot guarantee that services (office visits, phone consultations, laboratory testing or other testing) will be reimbursed. We do try to seek the best quality and prices for laboratory testing and nutritional supplements. You will be provided with diagnosis and procedure codes to assist you with possible insurance reimbursement.

Prescription Refill Requests and Supplement Orders
 
Hormone therapy prescriptions will be written to provide you with enough refills until your next scheduled office visit. If you are unable to come to your scheduled visit due to unforeseen circumstances, a one month refill may be issued (if deemed medically appropriate) to allow you to re‐schedule the missed appointment. If this re‐scheduled visit is missed, we will be unable to issue any further refills until you are seen in the office.

It may take up to 24-48 business hours to process a prescription refill or nutritional supplement order. Please plan ahead to avoid any interruption in your prescriptions or nutritional supplements. Prescription refill requests can be faxed to our office by the pharmacy. Occasionally, the supplement companies we use may have back orders on your items. We recommend activating a supplement or prescription refill request at least one week or more before you anticipate running out of what you need.
 
Laboratory Testing
 
We do not make any guarantees about insurance coverage for lab testing. If you have a PPO or Medicare some of your testing may possibly be covered. The amount of coverage depends on too many factors for us to be able to give you an exact cost or even a good estimate on how much you may have to pay. We do recommend that once you receive your laboratory requisition, you check with your insurance company to verify whether or not your tests are covered. We use a laboratory company that offers very low cash pay rates on tests that your insurance company will not cover or if you have a high deductible insurance plan. Some of the Functional Medicine specialty tests that we order may have a guaranteed price which will be discussed with you by the office staff after the doctor prescribes the test. If you have HMO insurance you will have to pay cash for your lab testing because we are out of your insurance network. We offer affordable lab testing options for low cash prices. We work very hard to give our patients affordable quality health care.
 
If you are paying out‐of‐pocket or have a high deductible on your insurance plan, please ask the front desk at checkout regarding obtaining discounted labs.
 
Follow up Visits
 
After your first follow up visit, your doctor will recommend when your next follow up visit should occur. In general, the second follow up visit for most may be at least within the following two to three months. The interval of follow up will depend on your individual needs and the complexity of your condition. After the first one or two follow up visits we require at least 6 month follow up intervals (or earlier if deemed medically necessary) to assess hormone levels and other essential tests. Your doctor also needs to assess the adequacy and safety of your treatment program.
 
In addition to follow up visits your doctor will need to perform a physical exam at least once a year. If you are prescribed hormones your doctor will also require a yearly prostate/rectal exam for men and a yearly gynecology exam for women.
 
These policies are in place to assure the safety and adequacy of your treatment program and are intended to help you achieve and maintain optimal health.
 
Medical Records
 
Sometimes your physician may need the benefit of reviewing some of your past medical records and studies you have had in order to determine the direction of your treatment program. Medical records can only be released to us with your permission. A medical records release form is enclosed with your new patient packet for your use. You are responsible for obtaining previous medical records from other physicians or health care providers. Please contact your physician or other health care provider to obtain these records. Your records should be mailed or faxed to our office.
